It depends on the model — each direct-vent fireplace publishes its own maximum total vent length and elbow allowance, and every elbow counts against that total. Longer runs, near the maximum, can slightly affect flame and draw, which is why manufacturers also require a minimum run and vent restrictor adjustments. There's no single 'X feet' answer, and anyone who gives you one is guessing. Every direct-vent gas fireplace ships with its own vent table that specifies a maximum total vent length for that exact model, along with how much horizontal versus vertical run is allowed and how many elbows you can use.
Elbows are the catch — each 45- or 90-degree turn eats up a chunk of your allowance, counting as several equivalent feet against the total. The coaxial pipe carries exhaust out and fresh air in through one assembly, and its reach is finite by design. The unit's manual is the authority.
Longer runs do change the burn — which is why there are minimums too
